Submission of this form does not guarantee approval or assignments. The Company may review the information provided and verify applicable requirements before approving a subcontractor.

1. Independent Subcontractor Status

The person signing this Agreement understands that they are applying to provide services as an independent subcontractor and not as an employee of My DMV Cleaning Services LLC.

The Subcontractor is responsible for operating their independent business and for complying with applicable federal, state, and local requirements relating to their business and services.

4. Required Documents & Verification

The following documentation may be required before the Company approves a subcontractor for assignments. The Company may request additional documentation when reasonably necessary to verify eligibility or compliance.

I understand that I may be required to provide a completed IRS Form W-9 for payment and tax reporting purposes. I understand that I may be required to provide applicable business registration or licensing information. I understand that proof of applicable insurance may be required when requested by the Company or required by applicable law. I agree to provide additional lawful business documentation reasonably necessary to verify my eligibility as an independent subcontractor.

Important: Do not upload Social Security cards, payment-card information, passwords, or other unnecessary sensitive personal information through this form.
